Member Forms

Did you know that in some cases, you may not need to download a form? We now have a secure self-service website: mySRPS. Members who have registered for mySRPS can make some transactions directly on line, no form needed.

We’re still at a very early stage with mySRPS. So right now, this applies to only a few forms. We’ve identified those in the sections below. When we add more features to mySRPS, we’ll update the information. Visit the page to learn more about mySRPS.

Note: You can print any of these forms and fill them out by hand. Some forms have another option. You can fill them out on line, then print them out with your information already entered. In either case, you must have a PDF reader. (If you need one, you can download Adobe Reader.)

You can also order forms by clicking here. Please note, it make take up to 3 business days for forms to be mailed.

Important! After you fill out the form, you must print it out and send it to us. The form itself tells you how to do this. Note that this is the case even if you fill out the form online. In some cases, you must get the form notarized when you sign it.

We cannot process your form until it arrives at our office in Baltimore.
