Member Forms

Did you know that in some cases, you may not need to download a form? We now have a secure self-service website: mySRPS. Members who have registered for mySRPS can make some transactions directly on line, no form needed.

 

We’re still at a very early stage with mySRPS. So right now, this applies to only a few forms. We’ve identified those in the sections below. When we add more features to mySRPS, we’ll update the information. Click here if you want to learn more about mySRPS.   

Note: You can print any of these forms and fill them out by hand. Some forms have another option. You can fill them out on line, then print them out with your information already entered. In either case, you must have a PDF reader. (If you need one, you can download Adobe Reader.) 

You can also order forms by clicking here. Please note, it make take up to 3 business days for forms to be mailed. 

Important! After you fill out the form, you must print it out and send it to us. The form itself tells you how to do this. Note that this is the case even if you fill out the form online. In some cases, you must get the form notarized when you sign it.

We cannot process your form until it arrives at our office in Baltimore.

Election to Not Participate in the Maryland State Retirement and Pension System

Note: Applies only to individuals who are eligible to elect to participate in the Optional Retirement Program or who are eligible to decline to participate in the Employees’ Pension System.

Teachers and Employees: Only eligible employees of community colleges, the University System of Maryland, Morgan State University, St. Mary’s College, and MHEC who may elect the Optional Retirement Program Form 60: Election not to Participate in the Teachers'/Employees' System by Faculty or Professional or Administrative Employees
Employees: Certain elected or appointed officials and others entitled to optional membership Form 60.15: Election not to Participate in the Employees' Pension System by an Elected or Appointed Official

 

Transfer of Service

Disability Application

Note: To apply for disability retirement, you must submit 3 forms:

  1. A Preliminary Application (Form 129)
  2. A Statement of Disability (Form 20)
  3. The Disability Retirement Estimate Form that applies to your system

Service Retirement Estimate

Note: If you’ve registered for mySRPS, you can create estimates on line – as many as you’d like. No forms, no waiting. You’ll get the results on the spot. You may download your estimates or just leave them on our site. (We’ll save them for you.) And you can create these estimates even if you’re many years from retirement.   2.  IRS Form W-4P for federal tax withholding​
  3. A Maryland State Tax Withholding Request (Form 766.11) if applicable
  4. A Direct Deposit Sign-up (Form 85)
